What to Bring

  • Your insurance card
  • Physician referral forms if required by insurance
  • A list of current prescriptions and/or over-the-counter medications you are taking, including dose and frequency
  • Pertinent information about your medical and surgical history
  • Any recent x-rays, laboratory tests, or appropriate records you may have

The best time to call for an appointment is between 9:00 a.m. and 4:00 p.m.

If you are a new patient to our practice, your initial examination, depending on the complexity of the problem, will take approximately one hour. Please try to arrive 15 minutes early to complete a patient registration form. Follow-up examinations are usually scheduled for 15 minutes to one-half hour depending on the nature of the problem and the length of time since the last visit.

We try our best to stay on schedule, although emergencies sometimes arise. If we are seriously delayed, we will try to notify you beforehand, if possible. We ask your patience if you have to wait; we know your time is valuable.

Appointment Cancellation

If you are unable to keep your appointment or are going to be late, please call the office as soon as possible. This courtesy allows us to be of service to other patients who are in need of our care.

Office Hours

The office is generally open from 8:00 a.m. to 5:00 p.m. Monday through Friday. After 5:00 p.m. during the week, on weekends, and on holidays, the answering service will take messages and relay them to the physician on-call. This service should be utilized only for urgent problems that need resolution prior to the next scheduled work day.

Prescription Renewal

If you need renewals of your current prescriptions, please call the office during normal working hours at least one week in advance. Please do not wait until you are out of medication before calling. Emergency requests for prescription refills after hours and on weekends will be limited to a one-week supply.

Telephone Calls

If you have any questions concerning your condition, medication, or treament, please call the office during normal working hours. Many of your questions can be answered by our staff; however, if you need to speak to one of the physicians, the receptionist will take a message and your call will be returned as soon as possible. Except for emergency situations, our physicians will not take telephone calls while treating other patients.

Emergencies

Our office provides coverage 24 hours-a-day, seven days-a-week. If you have an urgent problem, please call the office for instructions. If you call after normal working hours, our answering service will take a message and contact the on-call physician. Please try to keep your telephone line free when waiting for an emergency call back so that the on-call physician can reach you promptly.

Credit Policy

We hope you understand that our credit and collection policies are a necessary part of assuring the financial resources needed to maintain our practice.

Charges for medical services are due and payable at the time services are rendered. In the event that other arrangements are made beforehand, a bill will be sent to you with payment due upon receipt of the bill.

We are pleased to assist you in the preparation of your insurance claim form. If you have a health insurance plan other than those in which we participiate, please understand your health insurance policy is an agreement between you and your health insurance company. Your doctor's bill is an agreement between you and your doctor. You are responsible for payment of the bill regardless of the status of your insurance claim.

If circumstances should make it impossible for you to meet our credit term, we invite you to call or personally discuss this matter with our business office. This will avoid misunderstandings and enable you to keep your account in good standing. Except when hardship warrants otherwise, past due accounts are referred to a collection agency.

At the time of visit you must have a properly prepared referral from your primay care physician if required by your insurance plan. Failure to bring a referral form can result in your being financially responsible for all services provided. You are also responsible for all the appropriate copayment and/or deductible as specified by your insurance plan. Copayments must be paid at time of service.

If you have insurance other than those listed above, payment is reqested at time of service. We will be happy to submit the claim to your insurance carrier for you to expedite your reimbursement.
